How To Prepare

If you haven’t been referred by a doctor, contact all the physicians you’ve seen for your pain issue and have each one submit:

  • Your last three office notes
  • Your most recent blood lab work on file
  • All your radiology on file (x-rays, scans, MRIs, etc.) including images and reports, for the last 18 months
