Dr. Kochert Pain & Health respectfully requests payment in full at the time of service. We do accept nearly all forms of insurance. However, your policy might not cover some of the pain therapies performed here.
If you have medical insurance, we recommend you contact your provider to confirm the answers to these questions:
- Does your policy cover regenerative-injection therapies?
- Is Dr. Carolyn Kochert in or out of your plan network?
- What is your annual deductible and have you met it?
If You’re Covered
We will bill your insurance company and await its response to your claim before we bill you any uncovered balance—due within 30 days thereafter.
We will not collect anything from you the day of your appointment, unless your plan has a copay requirement, likely indicated on your insurance card.
NOTE: For a specialized practice like Dr. Kochert’s, copays are usually higher than those of primary care physicians.
If You’re Self-Pay
If you’re an uninsured self-pay patient, or your insurance does not cover your procedure, this rate card should be helpful.
